How can some people get lung cancer from smoking and some don't

How can some people get lung cancer from smoking and some don't?
Say two people have been smoking for 20 years... they smoke the same amount of cigarettes a day... yet one of them gets cancer and the other doesn't? I've heard tons of stories that people can smoke their whole lives and never get cancer, emphysema, etc, but some people smoke less and they get it?

There is no way in this world to answer this question. I am sure many a doctor would like to know this as well. My dad died from lung cancer but he had smoked all of his life. He died at 72. I have wondered the same thing that you asked.

Genetics, degrees of stress, age, other forms of pollution the person comes in contact with, eating habits--there are so many different factors involved that it would be hard to say.

There is a theory that smokers get a different type of lung cancer than non-smokers . . that the disease occurs in the lungs for both . .but that it is two distinctly different cancers.

Multiple reasons. For one thing, cancer is a statistical process -- multiple protective mechanisms have to be knocked out for it to occur. Think of the smoke as darts and suppose you have to get 10 darts in the same section of the dartboard to cause cancer. In some games, that's going to happen sooner than in others. For another, there are genetic differences, For example, they'e recently discovered a gene that causes some people to hold smoke in their lungs longer than others. The people who hold the smoke in longer have higher cancer rates, in fact they may account for most cases of cancer due to smoking. Susceptibility to emphysema, too, may be strongly influenced by genetics, and heart attacks. External factors can also interact with the smoke to increase the chance that disease will occur. Radon gas, for example, is after smoking the second-leading cause of lung cancer.
