Cancer eventually causes organ failure and then death.

Lung cancer eats up the lungs so they no longer get oxygen from the air they breath, and cancer can spread to other organs in the body, like the liver. You cannot live without your liver filtering toxins from the body. Stroke is a blood clot in the brain, so when a vessel gets clogged, the blood can't get to the brain tissue and the tissue dies. Sometime with a stroke, the vessel ruptures and again, the blood doesn't get to where it needs to go. In a heart attack a blood clot blocks the flow of blood to the heart muscle and the heart muscle dies, then it is no longer able to pump blood to the rest of the body.

my dad had cancer from smoking. as it progressed, the doctor increased the medication for pain. eventually, i'm pretty sure that the drug dose was high enough that he couldn't take it. so, in the end, the drugs, probably thorazine, killed him.
